Patient Records Transfer Notice

Alvarez Medical Center closed its physical location, effective March 31, 2026. SandBay Primary & Urgent Care recently acquired Alvarez Medical Center. As part of the acquisition, the team at SandBay is excited to welcome many of Dr Alvarez’s patients, but you will need to check with SandBay to see if they accept your insurance plan if you wish to transfer your care there. Dr Alvarez will see patients one day per week at SandBay Primary Care to help with the transition.

All patient records will be automatically transferred to SandBay Primary & Urgent Care Center. This means that if another medical office needs to request your medical records, they will have to contact SandBay, not Alvarez Medical Center.
